The study aims to quantify the rate of missing in-patient medical records classified by departments, wards, and cost of missing in-patient medical records at Sappasitthiprasong hospital in annual budget 2007. Data were collected from HOMC and CardTr programs which are databases for identifying discharged patients and for information related to adjusted relative weight (AdjRW) of diagnosis-related groups (DRGs) respectively. Costs of missing in-patient medical records were calculated from AdjRW of each patient and the average of AdjRW from all patients. The results show that, of 79,727 in-patient medical records, 69 (0.088%) medical records were missing. Forty seven (0.060%) and 22 (0.028%) in-patient medical records were missing at wards and medical doctors’ room respectively. Classified by department, the highest rate of missing in-patient medical records was at department of ophthalmology (0.190%). The second highest rate of missing in-patient medical records was at department of obstetrics and gynecology (0.176%) whereas the lowest rate of missing in-patient medical records was at department of surgery (0.029%). Classified by wards, the highest rate of missing in-patient medical records was at ophthalmology ward (0.143%). The second highest rate of missing in-patient medical records was at obstetrics and gynecology ward (0.103%) whereas the lowest rate of missing in-patient medical records was at surgery ward (0.021%). Cost of missing in-patient medical records calculated from AdjRW of each patient and from the average of AdjRW were 155,121.50 baht and 315,005.00 baht respectively.
