Abstract

Title : งานวิจัยนี้เป็นการวิจัยเชิงพรรณา เพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับการปฏิบัติตามกระบวนการวางแผนจำหน่ายผู้ป่วยของพยาบาลวิชาชีพ โรงพยาบาลสรรพสิทธิประสงค์ อุบลราชธานี กลุ่มตัวอย่างเป็นพยาบาลวิชาชีพทุกคนที่ปฏิบัติงานในหอผู้ป่วยและไม่ได้ปฏิบัติหน้าที่หัวหน้าหอผู้ป่วย โรงพยาบาลสรรพสิทธิประสงค์ อุบลราชธานี จำนวน 269 คน เก็บข้อมูลโดยใช้แบบสอบถาม ซึ่งประกอบด้วย 4 ส่วนคือ 1) ข้อมูลทั่วไป 2) การรับรู้บทบาท 3) ปัจจัยสนับ สนุนด้านนโยบาย การบริหารจัดการ การติดตามประเมินผล และ 4) การปฏิบัติตามกระบวนการวางแผนจำหน่าย ผลการวิจัยพบว่ากลุ่มตัวอย่างมีอายุเฉลี่ย 33.55 ปี ระยะเวลาปฏิบัติงานในตำแหน่งพยาบาลวิชาชีพเฉลี่ย 7.74 ปี เคยได้รับการอบรมเรื่องการวางแผนจำหน่ายร้อยละ 80.3 (เฉลี่ย 1.62 ครั้งต่อคน) หอผู้ป่วยที่ศึกษามี จำนวนพยาบาลที่ขึ้นปฏิบัติงานใน 1 วันเฉลี่ย 9.67 คนและร้อยละ 66.2 มีอัตราครองเตียงมากกว่า ร้อยละ 80 มีปัจจัยสนับสนุนด้านนโยบายในระดับปานกลาง การบริหารจัดการในระดับปานกลาง และการติดตามประเมินผลในระดับปานกลาง กลุ่มตัวอย่างมีการรับรู้บทบาทการปฏิบัติตามกระบวนการวางแผนจำหน่ายด้านต่างๆ ดังนี้ ด้านการประเมินปัญหาในระดับมาก ด้านการวินิจฉัยปัญหาในระดับปานกลาง ด้านการวางแผนการจำหน่ายในระดับมาก ด้านการปฏิบัติตามแผนในระดับมากและด้านการติดตามประเมินผลในระดับมาก กลุ่มตัวอย่างมีการปฏิบัติตามกระบวนการวางแผนจำหน่ายผู้ป่วยทุกด้านในระดับปานกลาง ปัจจัยที่มีความสัมพันธ์ทางบวกอย่างมีนัยสำคัญทางสถิติกับการปฏิบัติตามกระบวนการวางแผนจำหน่ายผู้ป่วย ได้แก่ การรับรู้บทบาทด้านการปฏิบัติ (r = 0.147, p = 0.016) ปัจจัยสนับสนุน ด้านโยบาย การบริหารจัดการและการติดตามประเมินผล (r = 0.394, p < 0.001; r = 0.416, p < 0.001; r = 0.430, p < 0.001; ตามลำดับ) ปัจจัยที่มีความสัมพันธ์ทางลบอย่างมีนัยสำคัญทางสถิติกับการปฏิบัติตามกระบวนการวางแผนจำหน่ายผู้ป่วย ได้แก่ อัตราการครองเตียง (r = -0.164, p = 0.007) ปัจจัยที่ไม่มีมีความสัมพันธ์กับการปฏิบัติตามกระบวนการวางแผนจำหน่ายผู้ป่วย ได้แก่ระยะเวลาการปฏิบัติงานในตำแหน่งพยาบาลวิชาชีพ การได้รับการอบรมเกี่ยวกับการวางแผนจำหน่าย จำนวนพยาบาลที่ขึ้นปฏิบัติงานและการรับรู้ด้านการประเมินปัญหา การวินิจฉัยปัญหา การวางแผนจำหน่าย และการประเมินผล

The purpose of this descriptive research was to determine the factors that correlated with the discharge planning process of professional nurses in Sappasittiprasong Hospital, Ubon Ratchathani Province. The subjects were all the 269 professional nurses besides the head of the hospital ward in. The questionnaire is composed of 4 parts: 1) general data 2) perception of roles 3) policy support factors, management and evaluation 4) the application of the discharge planning process. The results showed that the average age of the subjects was 33.55. Their average working experience span was 7.74 years. 80.3 percent of the nurses had training experience on discharge (1.62 times per person on average). The average number of nurses on duty in the hospital ward per day was 9.67. 66.2 percent had the bed occupation rate of more than 80 percent. The policy support factors, management and evaluation were on the moderate level. The subjects had the perception of roles concerning the application of the discharge planning process in the following area as follows: high level problem assessment, moderate level problem diagnosis, high level discharge planning, high level application of planning and high level evaluation. The subjects also had moderate level application of the discharge planning process in every area. The factors that positively correlated with the application of the discharge planning process with statistical significance were 1) perception of roles: (r = 0.147, p = 0.016), 2) policy support factors, management and evaluation: (r = 0.394, p < 0.001; r = 0.416, p < 0.001; r = 0.430, p < 0.001 respectively). The factor that negatively correlated with the application of the discharge planning process with statistical significance was the bed occupation rate (r = -0.164, p = 0.007). Finally, factors which did not correlate with the application of the discharge planning process included working experience, training experience on discharge, number of nurses on duty and the perception of problem assessment, problem diagnosis, discharge planning and evaluation.
