The purposes of this study were to explore the medication errors in process of medication reconciliation and to examine the associated factors. This cross sectional study was performed in 80 cancer patients who admitted at Chemotherapy ward, Sappasittiprasong Hospital during August 15th to September 23rd , 2011. Demographic data, medical history, social history, past medication history were collected from patients, caregivers, hospital database and medical chart. Identifying medication errors was done by compared past medication history and current medication during admission in hospital. Data was analyzed by using descriptive statistics, Chi-square test or nonparametric statistics where appropriate. The study found that the two most higher medication errors were omission error (7.5%) and wrong dose or frequency route (5%). Moreover, the associations of omission error and present of underlying diseases (p-value=0.039) were found statistical significance. The results have been shown the numbers of medication errors in process of medication reconciliation in cancer unit. In addition, the present of underlying diseases and cancer type are the associated which should be monitor during process of medication reconciliation in the future to prevent and decrease the medication errors that could harm the patients.
