This main purpose of this study was to develop a database of look-alike/sound-alike (LASA), commercially available in Thailand (Phase 3). Thailand and international data of look-alike/ sound-alike including LASA oriented-medication errors had been reviewed through standard pharmaceutical references for database, website and instruction manual development. The information was gathered by surveying LASA from hospitals in Thailand and the LASA database instruction manual was constructed. The information in database and website were composed of criteria for LASA category, LASA list, clinical significance, as well as severity of medication error. The content, feasibility, and overall user satisfaction of database were conducted in 78 hospital pharmacists. The satisfaction level of content, feasibility, and overall satisfaction were reported good 49.8%, 45.8%, and 39.0%, respectively. Moreover hospital pharmacists agreed that this database was functional and beneficial for pharmacy practice. However, it had been suggested that accessibility of data should be improved by transforming into a reading material or a hard copy which are more easily to be referenced.
