The purposes of this study were to identify the rate of post-dispensing errors and classify post-dispensing errors at outpatient pharmacy clinic at Buriram Hospital. Patients were randomly selected from out-patient pharmacy department during 10=00 AM-12=00 PM, 3 - 21 October 2011, except holidays. Data collection form was developed, validated and tried out in preliminary study for the final version. Data were descriptively analyzed. The result showed that 48 prescriptions of 256 (18.75%) were found at least one type of post-dispensing errors. Medication errors were identified in 132 of 1,015 drug items (13.00%). They were classified into one of the following type of medication errors= (1) 38 prescriptions of no drug counseling (14.84%), (2) one prescription of dispensing to wrong persons (0.39%). (3) one prescription of wrong number of items (0.39%), (4) one incorrect; dosage form, (5) one incorrect administration, (6) one incorrect medication, (7) one with expired medications and (8) one improper containers (each 0.10%), (9) two wrong dose and (10) two wrong strength (0.20%), (11) eight incorrect amount of drug items (0.79%). In conclusion, 18.75% of 256 prescriptions or 13.00% of all drug items were identified with at least one of those dispensing errors. The most frequently found dispensing error was no drug counseling (14.84%).
