Warfarin is a narrow therapeutic index drug. Due to its action can be affected by many factors, target INR is sometime difficult to control. This analytical, cross-sectional study was designed to determine clinical factors affecting on target INR control in outpatients warfarin clinic, Buriram Hospital. Clinical factor data from patients visiting at warfarin clinic and cardiovascular clinic during 1 September 1st,2011 to November 11th, 2011 were collected by questionnaire and medical record. Analyzing data was performed by descriptive statistics and logistic regression analysis.
A total number of 278 patients were enrolled in this study. The mean age of participant was 55.6 years old. Valvular heart disease was the most indication of warfarin (23%). Seventy-six percent of patient needed a target INR at 2.0-3.0 and target INR was achieved with 60.79%. The result from multivariate logistic regression analysis demonstrated that clinical factors related with target INR out of control were inappropriate warfarin dosing when compared with appropriate warfarin dosing (OR = 0.056; 95% Cl= 0.026-0.118, p<0.05) and non-compliance when compared with compliance (OR = 0.172; 95% Cl= 0.079-0.404, p<0.05). In conclusion, inappropriate dose and non-compliance were the significant clinical factors that impact on INR target control. These results can help to improve target INR control in warfarin-treated patient. |
