Chronic obstructive pulmonary disease (COPD) is a type of obstructive lung disease characterized by chronically poor airflow and not fully reversible. Thetreatment goal is toalleviatesymptoms, decrease the frequency and severity of exacerbations, increase exercise tolerance and quality of life. Objectives= To evaluate medical care outcome, evaluate medical treatment by disease severity and quantifyemergency room admission rate from exacerbation after patient receiving medical careat COPD Clinic, Sappasitiprasong Hospital. Materials and Methods= This is retrospective cohort study. The sample was69 patients with chronic obstructive pulmonary disease and received medical careat COPD clinic duringOctober 1st, 2010 to September 30th, 2012. Demographicand clinic data werecollected from OPD card and database called HomC. Results= By following individual patient 6 times in 1 year period, we found the complete data that can be used for evaluation 51 of 414 times (12.3%). There are 17 patients who hadthe complete data for evaluation ofmedical care. At the 1st visit at COPD clinic, 88.2% of patients received appropriate medical care compared todisease severity and 50%, 100%, 92%, 87.5% and 87.5% of appropriatemedical care were observed in the 2nd, 3rd, 4th and 5thvisit respectively. It was also found thatthe rate of exacerbation decreased from 0.99 time/person/year to 0.61 time/person/year. Conclusion= Patients who received appropriate medical care from COPD clinic, Sappasitiprasong Hospital had better disease controlled. However, the availability of data that were used for evaluating appropriateness of provision of medical carecategorized by disease severity as recommended by GOLD 2011was too low. This was due to during the study period, the data collection system was on developing process.
