Presently, the use of vancomycin is challenging due to the rising of prevalence of vancomycin resistant organismand itsadverse events.Objective: To review the usage of vancomycin in terms of indication, dosageand administration including efficacy and adverse effect.Method: The retrospective, descriptive study was performed by reviewing the database of Fort Sunpasitthiprasong Hospitalduring 1st of October 2012 to 30th of September 2015.Patients older than 18 years old treated with intravenous vancomycin were included.Results: From 30 patients included in this review,60%of them were male with themean age74.5±18years.Vancomycin was mostly prescribed with an indication ofarteriovenous fistula or arteriovenous graft infection (26.67%).Among 10 patients (33.33%) who were treated with empirical treatment, vancomycin was continuously given in only 1 patient after the negative culture had been reported. Almost of patients (90%) were treated within 14 days. The vancomycin was prescribed with a specified infusion period and volume of dilution for 50.00% and13.33%, respectively. Dosage adjustment in renal impairment was done in 40.00%.Adverse effect, such as renal function, was monitored during the use of vancomycin in about one-half of patient (26.67%).At the end of treatment, most of patients (86.67%) were cure or improved response. Conclusion: The usage of vancomycin in Fort Sunpasitthiprasong Hospital is generally appropriate. However, prescribing of vancomycin should be more carefully in dosage regimen, administration and adverse effect monitoring. The results of this drug use review are helpful to promote the drug safety and help in prevention of drug resistant organism.
