Drug dosing errors are common in renal impaired patients. Appropriate dosing adjustment is important to ensure patients’ safety and to avoid adverse drug effects or any poor outcomes.Objectives:To assess the rate of the potentially inappropriate dosing adjustmentin hospitalized patients with renal impairment for further useofrenal dose adjustment guidance. Methods: The prospective study was conducted in patients with renal impairment fromAugusttoOctober2016 at the regional hospital in Ubonratchathani province. Potentially inappropriate dosage adjustmentwas reported by using descriptive statistic.Results:1,235 patients werereviewed. The study found 180 patients(14.6%)with renal impairment and232medications were required dosage adjustment.The study also found that 207 (89.2%)medications were renally adjusted appropriately and 25 (10.8%) were inappropriately adjusted. The physicians’ acceptance rate was 48% (12 out of 25).Conclusion: The study identified a number of renal impaired subjects who received potentially inappropriate dosage regimen (13.9%).Two most common drug classes that were not renally adjusted appropriately are antibiotics andcardiovascular drugs.Therefore, the renal dosing guidance may be useful to avoid inappropriatedosage regimen and unwanted drug effects as well asto ensure optimal patient outcomes.
