Title : Quality of care indicators after modified process of pharmaceutical care in Diabetes clinic at Warinchamrab Hospital

Since year 2015, the process modification of pharmaceutical care has been implemented in diabetic clinic of Warinchamrab hospital. There have been changed from normal care after patients see their doctors to proactive pharmaceutical care in individual patients. Moreover, Clinical Practice Guideline of diabetic care was developed, announced and implemented in diabetic clinic. Objectives: For study and compare quality of care indicators after modified process of pharmaceutical care in 3 aspects including 1) Process of care indicators 2) Clinical outcome indicators 3) Drug Related Problem (DRPs) indicators. Methods: This study was retrospective study based on hospital electronic database. Quality of care indicators have been compared between year 2014 and year 2015. Data was analyzed by SPSS. McNemar test and Wilcoxon signed-rank test was used for statistical testing in categorical data and continuous data. Results: After modified process of pharmaceutical care found that process of care indicators were statistically significant improved (p<0.05) especially in percentage of foot examination, lipid profile examination, proteinuria screening and microalbuminuria patients who have got ACEIs or ARBs medications. Clinical outcome and DRPs indicators were statistically significant improved (p<0.05) especially in HbA1c FBS LDL TG HDL BMI. However, Three DRPs was found in the most frequent including failure to receive medication in aspect of non-compliance, adverse drug reaction and improper drug selection. Conclusion: As a result of modified process of pharmaceutical care, more DRPs in individual patients was resolved by pharmacists. There were improved in quality of care indicators including process of care and clinical outcome indicators.
