Title : The effects of applying INHOMESSS for home visits in diabetes patients by pharmacy students

the INHOMESSS was principal tool for holistic home visits in family medicine that consist of 4 aspects; Bio-Psycho-Social-Spiritual Objective: To study the effects of applying INHOMESSS for home visits in diabetes patients by pharmacy students in the community pharmacy course in the first semester, academic year 2015 Materials and Methods: documented reviewed of 43 diabetes patients who was provided pharmaceutical care using INHOMESSS. The analysis using descriptive statistics for quantitative data and content analysis for qualitative data. Result: Bio aspect, Hypertension was the most found comorbidity (18 patients, 41.86%). The average BAI score was 19.67 points, classified as the mild dependence group that has high ability to perform daily routine activities. Metformin combines with Glipizide was the most found medication-used (27.91%). The most frequently problems were Drug Related Problems (79.07%) especially in non-compliance (65.12%) followed by behavioral problems such as improper diet (62.79 %). These two problems has been solved and declined by next visited. Social aspect, most patients have social support, financial resource and power of attorney. However, there were also some patients who have service accessibility problems by financial problems. Phycho and Spiritual aspects, most patients (90.70 %) had a good attitude for their disease. Most patients (53.49 %) had the idea that diabetes was incurable but accepted that diseases can be controlled bycontinuous medication and good practice. In the other hand, there were 18.6 % of patients feel disgusted by their medications and disease status. 9.3% of patients were afraid of the consequences of the medications. Moreover, there was one patient who didn’t want to live longer (2.32 %). Conclusion: patient’s status in term of holistic care can be presented by applying INHOMESSS for home visits. However, there were some limitations such as ability to access patient’s clinical data information and ability to apply clinical knowledge integrate with INHOMESSS of students.
