Herbal medicines now globally use for both treatment and health promotion purposes. Co-administration of herbal medicine and medicine may cause metabolic herbal-drug interaction similar to a classic case i.e. grape fruit juice and cytochrome P450 (CYP) 3A4 substrate drugs (e.g. midazolam, terfenadine). This study was aim to survey for the possibility of Thai herbal-drug interaction in psychiatric out-patients of Prasrimahabhodi Psychiatric Hospital, Ubon Ratchathani. The random selected out- patients were interviewed and list of drugs used individually were collected from their prescriptions during April – June 2009. A total number of patients enrolled this program was 138 (75 were male and 63 were female). Majority of them were 31-40 years old (38.4%) and live in Ubon Ratchathani province (53.6%). Most of the patients came for Schizophrenia treatment (59.4%) and 10.9% of these patients also has diabetes mellitus and dyslipidemia. The interview results revealed that 37 patients (26.8%) used herbal medicine by themselves following their friends or relatives suggestions (62.5%) and for supportive treatment (20.0%). The herbal remedies used were 43 items i.e. Fah-talai (Andrographis paniculata) capsules (16.3%), Ya-Moe (a hot pot of several herbs) (9.3%) and fresh Fah-talai (7%). The 22 pairs of herb and drug (11.3%) have high chance to cause interaction i.e. Diazepam, Chlorpromazine and Haloperidol with Fah-talai. Thus both in vitro and clinical research of co-administrative of these psychiatric drugs and Fah-talai should be further investigated.
