This main purpose of study was to develop a database of look alike/sound alike (LASA), named UBU LASA Drug Database Version 1.0 for tablets and capsules commercially available in Thailand. This project was cooperation between Faculty of Pharmaceutical Sciences and Sapprasithiprasong Hospital. Thailand and international data of look alike/ sound alike tablets and capsules, including LASA oriented-medication error, had been reviewed through standard pharmaceutical references for database development. The database was composed of 2 parts. The first part was LASA list of tablets and capsules; LASA drug, criteria for LASA, medication error and problem solving, severity of problem, documentation / reporter, clinical significant, reported by Sapprasithiprasong Hospital and Medmarx®. The second part was designed for each individual user to create their own LASA database. Clinical significant and severity of problem could be identified by embedded program in the database. The content-feasibility and overall user satisfaction of database was conducted in 20 hospital pharmacists. The results of content-feasibility were excellence(2.00%), good (60.00%), and average (35.00%). The satisfaction for manual and installation of database was reported excellence (14.00%), good (67.00%), and average (37.00%). The overall user satisfaction, in terms of display attractiveness and practicality, was reported excellence (5.00%), good (53.00%), and average (33.00%).
