The aim of this survey research was to measure the incidence of drug-related problems (DRPs) during admission to hospital, transferring between wards, discharging to home and also during in hospital at internal medicine woman unit 1 at Suppasitiprasong hospital. Data was collected during April – May 2009 by reviewing data from 60 random samples of out-patient department card and in-patient charts. DRPs for medication reconciliation form was developed during a training period to evaluating drug-related problems from 15 charts. Three evaluators independently extracted and evaluated patients’ charts. Consensus method was used in case of disagreement.
Result found that a total 862 medications were prescribed. Thirty one DRPs were identified. Agreement between evaluators was 100%. Sixteen DRPs occurred during discharge. The most frequently found DRPs during discharge was untreated indications (n=10). Fifteen DRPs were found during patients were staying in ward. The second frequently found DRPs during patients’ treatment in ward was drug interaction (n=7). No DRPs occurred during admission to hospital. Therefore, hospital administrators should set a policy to monitor DRPs for patients who continuously receive medication and to solve DRPs immediately.
